A questionnaire is only as good as its questions. Design it well (AS91263) and it gathers clear, honest data to answer your investigative question; design it badly and every answer is biased or unusable before you even start. The skill is choosing the right question type and response options, wording each question so it is clear and neutral (no leading, loaded or double-barrelled traps), and putting it together with sensible order, privacy and a pilot test. Marks come from questions that are unbiased, easy to answer, and tied to the purpose.
